Western Tidewater CSB is seeking qualified Case Managers for our Substance Use Program.
**Substance Abuse and Mental Health experience is preferred for this position. **
Responsibilities and Duties for Case Manager:
Conduct comprehensive biopsychosocial assessments and screenings to determine client needs related to substance use disorder (SUD), mental health, housing, employment, and recovery supports. Develop individualized service plans in collaboration with clients, ensuring alignment with SOR program goals and evidence-based practices. Monitor progress and update service plans regularly to reflect client needs and treatment goals. Manage consumer records according to all Licensure and Medicaid standards. Coordinate referrals to appropriate treatment services, including detox, medication-assisted treatment (MAT), residential, outpatient, and peer recovery services. Assist clients in accessing wraparound supports such as housing, transportation, employment, food assistance, and medical/psychiatric care. Establish and maintain relationships with community providers to ensure warm handoffs and continuity of care. Work closely with clinical staff, peer recovery specialists, and other service providers to coordinate care. Qualifications and Skills for Case Manager:
BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field One year of clinical experience in a Human Services field QMHP-T (Trainee)
required QIDP or QMHP A (Adult) or C (Child)
preferred KSA’s
(Required) Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter
