Western Tidewater CSB is seeking qualified Support Coordinators and/or Case Managers for the Intellectually Disabled, Substance Abuse, and Mental Health population.
**On year Intellectual Disability experience is required for this position. Substance Abuse and Mental Health experience is preferred. **
Responsibilities and Duties for ID/DD Support Coordinator and Case Manager:
Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP Assist and provide follow up instruction, education, and counseling to consumers/families with ID/DD, SA, and/or mental health Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits Manage consumer records according to all Licensure and Medicaid standards. Qualifications and Skills for ID/DD Support Coordinator and Case Manager:
BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field One year of clinical experience in a Human Services field QMHP or QIDP
preferred KSA’s
(Required) Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter Benefits for Full-Time ID/DD Support Coordinator and Case Manager:
