CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Community Integrated Case Manager CBI is the premier non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education, and treatment using evidence-based models. Job Summary The Community Integrated Case Manager provides case management services in shelter or housing programs and acts as a liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. The role involves assisting patients and the treatment team in planning, connecting, outreaching, advocating, coordinating, and monitoring patient needs and progress. The case manager helps patients access health services, financial aid, employment, education, social services, and natural supports, while developing independent living skills. Responsibilities include assessing patient needs, developing and monitoring individualized treatment plans, and documenting services accurately and timely. CBI employees must embrace the recovery philosophy and promote a safe recovery environment. Skills/Requirements: High school diploma or GED required. Associates Degree or higher in a related behavioral health field preferred. Recovery from alcohol or drugs and/or GMH for at least 1 year preferred. At least 1 year of experience in behavioral health outreach and engagement activities. Or equivalent combination of education and experience. Valid AZ Driver's License and a 39-month MVR. Behavioral Health Technician (BHT) certification per A.A.C. R9-10-101.33 and CBI policies. Arizona Fingerprint Clearance card. Jail Clearance, if applicable.
