Discharge Planner - Case Management - Lakewood - FT Days at UC Irvine (UCI Health). Plan, manage, and coordinate the discharge plan for acutely hospitalized patients. Collaborate with the multidisciplinary team to coordinate care across the continuum and ensure safe and timely transitions. Utilize knowledge of available community resources and healthcare benefits to support the discharge process. Demonstrate excellent written and verbal communication skills and the ability to work in a high‑intensity environment. Qualifications
Required Qualifications: Previous acute care, healthcare, or related experience. Must work every other weekend. Ability to demonstrate strong customer service skills and maintain an appropriate work pace. Excellent written and verbal communication skills in English. Ability to establish and maintain effective working relationships across the health system. Knowledge of regulatory guidelines as applicable (Medicare/CMS, TJC, NCQA) is preferred. Preferred Qualifications: One year or more of acute care hospital discharge planning experience. CA RN or LVN license. Knowledge of UCI policies, procedures, and organizational structure.
