We are seeking a

Sr. Clinical Social Worker

who will provide social work case management services to patients of the John G. Bartlett Specialty Practice to reduce obstacles to care and to increase adherence. Supervises assigned staff and oversee daily case management activities. Specific Duties & Responsibilities Clinical Identify patient biopsychosocial needs and develop plan of care in collaboration with patient and multidisciplinary team. Provide mental health education and counsel patients to enhance psychological and physical health of the patients while promoting behavioral change. Refer patients to hospital and community resources and coordinate care with community-based case managers. Assess patients for Ryan White eligibility funding. Provide case management services for walk-In patients. Perform biopsychosocial assessments of new patients entering HIV care. Provide crisis intervention services when required. Complete appropriate documentation in the Epic medical record system. Participate in relevant training, clinical supervision, conferences and meetings. Administrative Provide supervision to clinic’s Medical Case Managers and complete annual evaluations. Organize staff competency and compliance trainings. Complete chart audits to review staff documentation of patient encounters. Coordinate monthly case management staff meetings. Provide clinical supervision for social workers seeking licensure through the state of Maryland. Train new hires to the case management team and involved in the onboarding process. Train multidisciplinary team members on services provided by case management. Serve on work groups to improve clinic workflow and patient satisfaction. Resolve patient complaints. Approve and track Ryan White vouchers for co-pay, medical, rent, utility, and other assistance provided to Ryan White patients in need. Track case management attendance records and approve time off requests. Special Knowledge, Skills, & Abilities Demonstrate general knowledge of HIV disease, treatment, resources; associated mental health and psychosocial support needs. Skilled in dealing with issues related to substance abuse, human behavior, crisis intervention, family dynamics, stages of development and the grief process. Knowledge of community-based service delivery systems. Knowledge specific to public entitlement programs related to health care. Skilled in collaboration with multidisciplinary teams. Skilled in biopsychosocial assessment and cultural elements of clinical social work practice. Knowledgeable about the current health care environment. Capacity to gather and synthesize relevant data. Skilled in crisis intervention, psychiatric evaluation, and substance use assessment. Able to work with culturally diverse patients, staff, and community populations. Strong organizational, writing, and verbal communication skills. Strong attention to detail and follow-through. Must be able to provide consultative services to case managers on the social work/case management team. Must be able to move between multiple tasks while balancing priorities in an unpredictable environment which requires a high level of flexibility and responsiveness.

Minimum Qualifications MSW from accredited university required. LCSW-C License required. Two or more years of case management experience in a healthcare or public welfare setting with knowledge of HIV, substance abuse, and psychiatric illness required.

For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.
