The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with healthcare teams and community resources to facilitate safe discharges and comprehensive care.
Assist patients and families in understanding and managing emotional, personal, and biopsychosocial issues impacting care and hospitalization.
Conduct assessments, provide crisis intervention, and coordinate referrals.
Work with multidisciplinary teams to develop discharge plans across the continuum of care.
Offering:
$3,000 sign-on bonus
Requirements:
Master’s in Social Work, LCSW license, current BLS certification, and preferably one year of clinical experience in a medical setting.
Pay:
$40.00 - $48.10/hour
