Our Community Stabilization Program supports individuals transitioning from acute behavioral health crises back into the community. The service aims to reduce hospitalization, improve continuity of care, and support clients in stabilizing their environment and functioning.

Position Summary

The Qualified Mental Health Professional (QMHP) will provide short-term, intensive community-based support to clients recently discharged from emergency, inpatient, or crisis settings. This position focuses on bridging individuals to long-term services, offering care coordination, skill-building, crisis prevention, and treatment planning. The QMHP will work with clients in homes, shelters, or other transitional settings. This position is available as a full-time, part-time, or contract role and is ideal for professionals seeking flexible scheduling or supplemental work opportunities.

Compensation

The pay range for this position is between $24 and $35 per hour, depending on experience and qualifications. Quarterly bonuses are available based on productivity and performance. Compensation structure can be tailored for W-2 employees or 1099 independent contractors depending on the agreement.

Key Responsibilities

Deliver person-centered, recovery-oriented community stabilization services to eligible clients.

Conduct intake assessments, strength-based evaluations, and safety/crisis plans.

Assist clients in accessing housing, employment, healthcare, and long-term mental health supports.

Provide psychoeducation and skill-building to improve daily functioning and independence.

Coordinate discharge planning and aftercare linkage with hospitals, crisis units, and outpatient providers.

Document services and maintain accurate clinical records per DMAS and DBHDS standards.

Participate in treatment team meetings, supervision, and outcome tracking.

Work in the field throughout the week to meet clients in their natural settings.

Qualifications

Bachelors degree in Human Services, Social Work, Psychology, or a related field (Required)

Must meet Virginia DBHDS qualifications as a QMHP-A or QMHP-Trainee

Minimum of 1 year of experience working with adults with mental illness (Preferred)

Understanding of crisis response, case management, and recovery principles

Strong clinical documentation and communication skills

Valid Virginia drivers license, reliable transportation, and willingness to travel to community locations

Ability to work flexible hours to meet client and program needs

Preferred Competencies

Familiarity with Medicaid community stabilization service requirements

Experience with community-based mental health services

Training in trauma-informed care and motivational interviewing

Electronic Health Record (EHR) documentation experience

Bilingual or multilingual skills (preferred but not required)
