About the Role The Family Physician Assistant plays a crucial role in delivering comprehensive healthcare services to patients of all ages within the Long Island Select Healthcare Inc. facilities. The PA collaborates closely with physicians to assess, diagnose, and treat a variety of medical conditions, ensuring high‑quality, patient‑centered care. The PA educates patients about health, preventive care, and chronic condition management, promotes overall wellness, and maintains accurate medical records to support continuity of care.
Minimum Qualifications
Bachelor's degree in Physician Assistant studies or a related field.
Current certification as a Physician Assistant (PA‑C) and valid state licensure.
Preferred Qualifications
Experience in family medicine or primary care settings.
Bilingual proficiency in English and Spanish or other languages.
Responsibilities
Assess and diagnose patients presenting with various clinical problems and complaints.
Provide education on the Patient Centered Medical Home (PCMH) and Chronic Care Management (CCM) programs, including brochures that describe program values and goals.
Obtain Medically Assisted Treatment waivers for patients with substance use disorders within 60 days of hire.
Become proficient in telemedicine and provide care via this platform as required.
Assist in the educational growth of the agency by allowing students to shadow as needed.
Participate in committees such as Quality Assurance, Peer Review, and Root Cause Analysis to improve workflow.
Develop, implement, and evaluate plans of care utilizing sound clinical judgments based on assessment of the physical, psychological, and emotional needs of clients.
Consult, refer, and collaborate with other disciplines involved in total patient care.
Document all activity using appropriate medical record forms.
Participate in ongoing educational programs to maintain clinical skills and proficiencies.
Skills Strong clinical assessment abilities, effective communication skills, organizational skills, and proficiency in electronic health record (EHR) systems are essential. Preferred skills include bilingual proficiency to connect with a diverse patient population.
Physical Demands The PA must routinely use hands to finger, handle, or feel and talk or hear. The PA frequently stands, walks, and sits, occasionally lifts up to 25 pounds. Reasonable accommodations may be made for individuals with disabilities.
Travel This PA may be required to travel between clinic sites depending on scheduling needs.
Work Environment The work environment is moderate in noise. The PA may come into contact with human blood or other potentially infectious materials (Category I).
