As a team member with Lakeview Terrace, you will provide services to our residents that call LVT home. Facility Name and Address: lakeview Terrace Retirement Community 331 Raintree Drive Altoona, FL 32702 Purpose: Assist with oversight and monitor staff development, ensuring that training and competency requirements for Lakeview Terrace are met per regulatory guidelines. Ensures patient care services are provided in accordance with acceptable standards of care. Ensures access to appropriately qualified support twenty-four (24) hours a day, seven (7) days a week. Act as Facility Infection Control Preventionist and ensure Trending and Tracking of Infections are monitored each month. Communicate with Director of Nursing and QUAPI team to identify negative trends and implement Performance Improvement Plans as required. Coordinate with LPN Unit Manager to present data and trends to QUAPI team to develop and implement best practice infection prevention and control guidelines as required by the Center for Disease Control and World Health Organization. Maintain monthly infection log and present to QUAPI meeting quarterly. Identify trends and report to Florida Department of Health and Center for Disease Control as required. Collaborate with Human Resources and Director of Nursing in coordinating Orientation and Annual trainings. Meet with all new hires and ensure on-boarding is initiated through Relias and assist new hires in the course work completion process. Notify Director of Nursing and Human Resources of insufficiencies in orientation Provide designated clinical and non-clinical trainings for current employees. Coordinate and train key sessions, annual trainings, as well as coordinate with Director of Nursing requests for specialized trainings. Revise and adapt as necessary. Assist all new hires with PCC access. Provide oversight of training and documentation of Competency Skills Verification with all nursing personnel and Nursing Assistants upon hire. Ensure any deficiencies are addressed and retraining provided. Assist Director of Nursing with interview process as needed. Assist state monitoring agencies to obtain unrestricted access where applicable to electronic health records, training, and personnel records to support a successful survey. Each member of the nursing administration team will cover one week of on- call coverage as assigned within the team rotation. On call duties will not be more frequent than once every four weeks as assigned, coverage to begin at 08:30 am clinical meeting each Monday upon acceptance of the on call phone and schedule book and will conclude on Monday of the following week at the 08:30 clinical meeting. All weekend and staffing issues to be addressed in on- call hand off report. Assist the Director of Nursing and provide oversight in the following areas: Physician rounds, daily chart audits and wound care, staff training, daily med cart inspections, daily staffing requirements, admissions, daily weights and vitals. Coordinates with Director of Nursing and Human Resources to ensure BLS/ CPR certification for all LVT employees, act as primary site CPR instructor in coordination with Director of Nursing and LPN Unit Manager Monitor and audit resident medical charts on Rehab Unit to ensure admissions are completed, orders are taken off and entered correctly into PCC, and interdepartmental notifications have been submitted for continuance of patient care. Act as a liaison between office staff, nurses, families and physician to ensure client receives quality care in an efficient manner. Identifies opportunities to improve the quality of patient care. Creates plans of care and clinical solutions to patient problems using the nursing process and critical thinking skills. Ensure compliance with all federal and state rules and regulations. Complete other duties as assigned by supervisor. Assist with other duties as assigned within the Nursing Department. Required Qualification: Registered Nurse with a current, valid, and unrestricted nursing license in the state of Florida required. Current American Heart Association Healthcare Provider Basic Life Support Certification required. Minimum of 2 years’ experience working with a variety of components of PointClickCare Software. Must possess a high degree of computer literacy. Must demonstrate excellent interpersonal and problem-solving skills to support nursing practice. For purposes of effective communication with patients, families and clients must be able to see, hear, speak, read and write English Specialized training in infection prevention and control required within 30 days of hire to meet criteria for Infection Preventionist. Maintain ability to stretch, push, pull, kneel, reach, twist, lift unassisted at least 50 lbs. and walk, stand, or sit up for up to eight hours per work day. Preferred Qualifications: Minimum of 2 years’ direct experience in a clinical and managerial role in a skilled nursing facility or other nursing/clinical role preferred, such as Charge Nurse, Unit manager, or Staff Development Coordinator. Experience with training and staff development preferred. American Heart Association Basic Life Support Instructor certification preferred or able to obtain within sixty days of hire Hourly Rate: Hourly Rate
