Overview
LPN Clinical Team Coordinator Home Health – Elara Caring. The Licensed Practical Nurse Clinical Coordinator (LPN CC) or Licensed Vocational Nurse Clinical Coordinator (LVN CC) supports Clinical Team Managers in planning and implementing quality care by advancing key workflows throughout the patient journey. This position monitors the accuracy and documentation of Homecare Homebase (HCHB) inputs and follows up to ensure workflow requirements are satisfied. The LPN/LVN CC also acts as a key intermediary between the patient, the CTM, the patient’s physician or medical provider, and other facility staff overseeing patient care.

Support Clinical Team Managers in planning and implementing quality care across the patient journey. Monitor accuracy and documentation of Homecare Homebase (HCHB) inputs and ensure workflow requirements are satisfied. Serve as an intermediary between patient, CTM, physicians/medical providers, and facility staff to oversee patient care.

What is Required?
Graduate of an accredited school of professional nursing Current Certified Licensed Vocational Nurse or Licensed Practical Nurse 1 year of home care or hospice experience Ability to seek patient feedback, advocate for patients, understand patient needs, and manage patient expectations English and Spanish bilingual preferred Valid driver’s license and insurance; reliable transportation
