The Hospice RN Case Manager is a professional nurse who coordinates and directs the hospice patient’s care in their home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs. Responsibilities include: Independently manage the hospice patient population with advanced assessment, teaching, and decision-making skills. Evaluate patients for appropriateness of hospice care and develop the plan of care in collaboration with the provider, caregivers, and Hospice IDG members. Provide follow-up by evaluating the effectiveness of the plan of care and monitoring patient and caregiver responses. Deliver patient care based on provider orders, agency policies, regulatory guidelines, and best practices. Collaborate with the provider, patient, caregivers, and Hospice IDG members to meet goals of care considering patient limitations and preferences in line with hospice philosophy. Document thoroughly and timely in the patient’s medical record according to agency requirements. Attend and actively participate in IDG and team meetings. Perform on-call responsibilities and Holiday staffing. What We Offer

Required Education:

Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing. Preferred Education:

Bachelor’s Degree in Nursing. Required Skills:

Nursing skills as defined by generally accepted standards of practice; ability to care for the clinical needs of the population served; professional and efficient verbal and written communication; good interpersonal skills; ability to multi-task; basic organizational and time management skills; basic computer skills; familiarity with general office practices; ability to work with patients and caregivers dealing with life-threatening illness and end-of-life; ability to prioritize multiple critical tasks. Preferred Skills:

Home Care Home Base experience; experience with smartphones and smart devices. Required Licensure/Registration/Certification:

Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas driver's license; Current auto insurance. Reliable transportation required. Preferred Licensure/Registration/Certification:

Hospice and Palliative Care certified; Wound Care Certified; advanced certification in area of specialty. Required Experience:

Two years’ experience in a healthcare setting in the respective discipline. Preferred Experience:

Medical-Surgical experience; experience in Case Management; supervisory experience in a healthcare setting; Home Care or Hospice experience. Job Details
