The Hospice Certified Nurse Aide (CNA) will provide personal hygiene care and assistance with Activities of Daily Living (ADLs) as per the patient’s plan of care in the patient’s home environment. The CNA will provide care based on provider’s orders, plan of care, departmental policies and procedures, regulatory guidelines and best practice. The CNA works under the supervision and direction of the Registered Nurse. The CNA works to assist in providing a safe and clean environment. The CNA collaborates with the patient, caregivers and other Hospice IDG members to meet the goals of care. The Hospice Certified Nurse Aide collaborates with the patient, caregivers and other Hospice IDG members to meet the goals of care with the consideration of the patient’s limitations and personal preferences. The Hospice Certified Nurse Aide participates in Holiday staffing. Other Items Of Note

Current Texas state certification as a Certified Nurse Aide Preferred Education

High school diploma or equivalent Required Skills

Certified Nurse Aide skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Preferred Skills

Home Care Home Base experience; Experience with smart phones and smart devices Required Licensure/Registration/Certification

Current Texas state certification as a Certified Nurse Aide; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation. Preferred Licensure/Registration/Certification

Advanced education or certification in area of specialty Required Experience

One year experience as a hands on caregiver Preferred Experience

Home Care or Hospice experience; Experience in healthcare setting Seniorirty level
