Overview Peterson Health is an independent, not‑for‑profit rural community hospital located in Kerrville, TX, heart of the Texas Hill Country. This PRN overnight position (5:00 pm–8:00 am) is on‑call and triage or visit as needed. The role supports hospice patients in their home environment.

Responsibilities
Independent management of hospice patient population using advanced assessment, teaching and decision‑making skills.
Evaluate patients for appropriateness of hospice care and develop plan of care in collaboration with provider, caregivers and Hospice IDG members.
Follow up by evaluating effectiveness of plan of care and monitoring patient and caregiver response.
Provide patient care based on provider’s orders, departmental policies, regulatory guidelines and best practice.
Document in medical record in a thorough and timely manner according to agency requirements.
Attend and actively participate in IDG and team meetings.
Perform on‑call responsibilities and holiday staffing.
Qualifications
Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.
Required skills: nursing skills, basic knowledge, efficient verbal and written communication, interpersonal skills, multi‑task, basic organizational and time management skills, basic computer skills, general office practices, ability to be open, sensitive, flexible and ecumenical, experience with life‑threatening illness and end of life.
Preferred education: Bachelor’s Degree in Nursing.
Required licensure/registration: Current Texas state license; Current American Heart Association CPR; Current Texas state driver’s license; Reliable transportation.
Preferred licensure: Hospice and Palliative Care certified; Wound Care Certified; Advanced education or certification in area of specialty.
Required experience: Two years’ experience in healthcare setting in most discipline.
Preferred experience: Medical‑Surgical experience; Case Management experience; Supervisory position in healthcare setting; Home Care or Hospice experience.
