Clinical Pharmacist - Regional Oncology and Infusion

A caregiver in this role will be based at Fairview Hospital but cover PTO and call-offs on site to support primarily west region oncology infusion centers. Cleveland Clinic’s inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians.
Pharmacists are responsible for knowing the purposes, effects, and side effects of various drugs. They should be able to assess a patient’s profile to determine if it is safe to combine new medications with medications the patient is already taking. Modern drugs can improve quality of life, but must be taken correctly, with proper frequency and dosage.
A caregiver in this position works on a hybrid regional schedule, Monday through Friday 7:30 pm – 4:00 pm or 8:00 am – 4:30 pm. This caregiver will provide coverage on site or float throughout the regional oncology and infusion locations.
A caregiver who excels in this role will:
Provide safe, efficient, patient-centered, evidence-based medication services.
Verify medication orders and/or dispense medications accurately according to state and federal regulations.
Document services in a complete, accurate, and timely manner.
Ensure all pharmacy operations comply with national accreditation standards.
Maintain professional interactions with patients to facilitate successful patient experiences.
Educate and train students, residents, other caregivers, and/or patients.
Minimum qualifications for the ideal future caregiver include:
PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
Current Basic Life Support (BLS) certification through the American Heart Association and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days (for individuals required to administer immunizations).
Current Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association. If caregiver responds to adult or pediatric cardiopulmonary arrest, this is required within one year of hire.
Completion of a postgraduate year 1 residency (PGY1), fellowship, or 2 years of relevant post-graduate pharmacy experience.
Post-graduate year 2 residency (PGY2) or relevant experience for clinical pharmacists hired in a specialized pharmacy practice area (e.g., Oncology).
Preferred qualifications for the ideal future caregiver include:
Oncology infusion experience preferred
Understand sterile product and compounding practices

Ability to stand and walk for approximately 75% of the shift and continuously for extended periods.

Normal or corrected vision.
Ability to clearly communicate verbally by phone or in person.
Personal Protective Equipment :
Follows standard precautions using personal protective equipment as required.
