Overview
Physical Therapist - Home Health - Full Time First Shift at Valley Health. The Home Health Physical Therapist provides direct patient care in the patient's home environment as part of a multidisciplinary team including nursing, physical therapy, occupational therapy, speech, social work and home health techs. The Physical Therapist evaluates patients and uses professional judgment to design the individualized plan of care which best meets the patient's needs. The Physical Therapist provides direct, one-on-one patient care, and may supervise care provided by assistants. The Home Health agency relies heavily on technology; email is a primary means of communication, and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies across acuity, age, and diagnosis. The Physical Therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator. Employment begins with a 4–6-week orientation with a seasoned preceptor to ensure the clinician is well prepared when they begin treating on their own. Additional duties include performance improvement opportunities, policy and procedure review and development, and research. Working in home health provides variety of settings and clinical challenges, offering the opportunity to work within the real-life environments of our patients." Responsibilities
Provide direct, one-on-one patient care in the home health setting as part of a multidisciplinary team. Evaluate patients and design individualized plans of care that meet patient needs. Supervise care provided by assistants as appropriate. Utilize technology for documentation and communication (laptop, electronic records, email). Participate in performance improvement, policy and procedure review, and research activities. Complete orientation successfully with a preceptor during the initial 4–6 weeks. Engage with patients across a range of acuity, ages, and diagnoses. Education & Experience
Bachelors, Master's, or Doctoral degree in Physical Therapy is required. A completed CI credentialing course, home study adult learning course, or equivalent is required. Minimum two years of clinical experience (4,160 hours) is required. Certification & Licensure
Physical Therapy license or eligibility in the state of assignment (Virginia or West Virginia) is required. Licensure in a secondary state may be required depending on geographical area. BLS Certification (American Heart Association Healthcare Provider) is required. New hires must have AHA certification prior to completion of orientation. Valid driver's license and reliable transportation is required. Qualifications
Basic computer and keyboarding skills or willingness to train within the first year. Ability to interact competently with patients according to age and needs. Access to transfer electronic data. No convictions or pending charges of barrier crimes. Benefits
