Essential Functions
Assist the Director of Clinical Services in completing initial, quarterly and annual resident assessments, and all other required service plans and assessments. Assist the Director of Clinical Services in scheduling staff, maintaining staff coverage, and supervising supportive living staff. Follow all standards regarding safety, licensing, regulatory and agency requirements. Responsible for maintaining resident files in accordance with HFS requirements, completing resident care plans. Assist the Director of Clinical Services in medication set up and dispensing; communicate with pharmacy regarding resident medication and concerns.
Any Additional Functions/Responsibilities
Work in conjunction with the Dir. of Clinical Services to perform on-call service as scheduled; respond promptly to emergencies; and provide shift coverage as needed. Work with Food Service Manager to provide input for meal plans Work with Activity Coordinator to plan and schedule appropriate activities for residents. Performs other related tasks as needed.
Position Qualifications
High school diploma or equivalent preferred; at least 18 years of age. Complete a Two-Step Tuberculin Test. Pass Health Care Worker Background Check. Be willing to participate in required training as necessary to meet requirements of the Supportive Living program. Background check clearance and Health Care Worker background check clearance required. Completion of a two-step Tuberculin test required. LPN, licensed by the State of Illinois, required. Certified in emergency resuscitation.
Job Knowledge, Skills & Experience
Must be at least 18 years of age or older. Minimum of three (3) years’ experience in healthcare services to adults with disabilities or the elderly population in assisted living, inpatient hospital, long-term care setting, adult day care, or in a department approved health related field. Minimum of one (1) year experience in a supervisory/managerial role required. Demonstrated ability to communicate in a clear, comprehensible manner, verbally and written. Excellent communication, organization, presentation and pc/computer skills (including working experience with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
