September 2025 Overview:

The RN Wound and Foot Care provides quality patient- and family-centered care as a member of the health care team, applying standards for professional nursing practice and focusing care to enhance the experience for patients and families. The position may oversee the work of other caregivers for assigned patients during the shift, as required under the scope of RN licensure. This position does not directly manage any other caregivers. Essential functions and duties

Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques and applies this knowledge in accordance with St. Charles Health System policies and applicable laws and regulations. Provides wound and ostomy care for patients per direction of the certified wound and ostomy nurse, including wound care assessment and implementation of interventions, communication with the care team, and participation in education activities promoting wound healing and preventing skin breakdown. Assesses the patient’s condition and needs using critical thinking and clinical judgment; sets outcomes, implements appropriate actions, collaborates with other disciplines, and evaluates patient progress. Maintains a safe environment for caregivers, patients and guests. Participates in continuous quality improvement and departmental activities and supports organizational goals. Participates in professional growth and development of self and others. Communicates effectively (written and oral) and maintains professional relationships with caregivers, medical staff, patients, and guests. Identifies work-related problems with possible solutions and implements solutions within scope of practice when appropriate. Coordinates and clarifies the interdisciplinary team’s roles to manage the patient’s experience. Performs stand-by call as assigned; varies by department. Supports the organization’s vision, mission and values; promotes Lean principles and continuous improvement. Delivers services with professionalism and confidentiality; complies with applicable laws, regulations, policies and procedures and reports known or suspected violations. Delivers patient care in a timely, efficient and accurate manner and may perform additional duties of similar complexity as required or assigned. Contributes to a healing environment and fosters intentional relationships to enhance the care experience. Provides therapeutic presence and patient-centered communication, including introductions, role explanation, respect for patient preferences, active listening, and appropriate touch and body language. Calculates dosages and administers medications accurately following organizational policies and procedures. Additional job duties

RN Certified Foot Care —Wound and foot and nail care and oversight for patients referred to the Wound & Ostomy department, including assessment, interventions, physician consultation, team communication, and education on foot and nail care. RN Certified Wound Care —Wound and ostomy care oversight, including assessment, interventions, physician consultation, team communication, and education to promote wound healing and prevent skin breakdown. RN Certified Wound Ostomy —Wound and ostomy care oversight, including ostomy consultations, stoma site markings, physician consultation, team communication, and education on wound and ostomy care. Education and licensure

Education:

Required: As required by RN licensure. Preferred: Bachelor's degree in Nursing. Licensure/Certification:

Required: Current valid Oregon RN license, valid Oregon driver’s license and ability to meet driving requirements. AHA Basic Life Support for Healthcare Providers. Foot Care Nurses Certification through AFCNA is required for any nurse providing foot and nail care. Preferred: Board Certified Wound Care or Ostomy Care Certification (ABWM or WOCNCB). Experience:

Required: One (1) year experience in an acute wound care setting. Preferred: N/A Skills and physical requirements

Excellent communication skills and ability to interact with a diverse population; professional representation of SCHS. Computer experience including electronic medical records and MS Office; strong teamwork and collaboration skills; ability to multi-task and work independently; attention to detail; strong problem-solving and decision-making skills; strong organizational abilities. Physical requirements: standing/walking and keyboard use most of the workday; ability to lift 1-25 pounds regularly, 25-50 pounds occasionally, with safe handling; PPE as required; ability to wear PPE and operate in various care environments. Schedule and environment

Schedule: 20 hours per week; Monday–Friday; First shift 08:00–16:30. Caregiver type: Regular Exemption status: Non-exempt Job family: Non-Contract RN
