Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques and applies this knowledge in accordance with St. Charles Health System’s policies and work instructions and within laws and regulations governing nursing practice in the State of Oregon. Provides wound and ostomy care for patients per direction of the certified wound and ostomy nurse; wound care assessment, implementation of wound care interventions per supervision of the certified wound and ostomy nurse, communication and collaboration with the care team, and support for organizational education activities promoting wound healing and preventing skin breakdown. Assesses the patient’s condition and needs using critical thinking and clinical judgment: sets outcomes; implements appropriate nursing actions and collaborates with other disciplines to meet physical, emotional, spiritual, social, and intellectual needs; evaluates progress. Provides and maintains a safe environment for caregivers, patients and guests. Supports department goals and participates in continuous quality improvement and departmental activities. Participates in activities that promote professional growth and development of self and others. Communicates effectively (written and oral) and maintains professional relationships with caregivers, medical staff, patients, and guests. Identifies work-related problems with possible solutions and implements solutions within scope of practice when appropriate. Manages the patient’s experience by clarifying, integrating, and coordinating the roles of the interdisciplinary team. Performs stand by call as assigned; varies by department. Supports the vision, mission and values of the organization; supports Lean principles of continuous improvement as a champion of change. Delivers customer service and/or patient care in a manner that is timely, efficient and accurate. May perform additional duties of similar complexity within the organization, as required or assigned. Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver. Participates in creating intentional relationships and demonstrates focus attitudes and behaviors that enhance the care experience. Provides a therapeutic presence in service to others by responding to patient needs in a caring way (introducing oneself, explaining role, asking preferred name, sitting with the patient to determine care goals, active listening, and effective communication). Calculates dosages and administers medications accurately in accordance with organizational policies and procedures for medication administration. Additional Job Duties

RN Certified Foot Care

– Responsible for wound and foot & nail care and oversight for patients referred to the Wound & Ostomy department, including assessment and interventions, physician consultations, care-team communication, and support educational activities for wound healing and foot/nail care. RN Certified Wound Care

– Responsible for wound and ostomy care and oversight for patients referred to the Wound & Ostomy department; wound care assessment, interventions, physician consultations, care-team communication, and educational activities promoting wound healing and preventing skin breakdown. RN Certified Wound Ostomy

– Responsible for wound and ostomy care and oversight for patients referred to the Wound & Ostomy department; wound care assessment, interventions, ostomy consultations, stoma site markings, physician consultation, care-team communication, and educational activities promoting wound healing and ostomy care. Education, Licensure and Certification

Education

– Required: As required by RN licensure. Preferred: Bachelor’s degree in Nursing. Licensure/Certification/Registration

– Required: Current valid Oregon RN license. Valid Oregon driver’s license and ability to meet driving requirements. AHA Basic Life Support for Healthcare Provider. Foot Care Nurses Certification through AFCNA is required for any nurse providing foot and nail care. Preferred: Board Certified Wound Care or Ostomy Care Certification through ABWM or WOCNCB. Experience

– Required: One (1) year experience in an acute wound care setting. Preferred: N/A Personal Protective Equipment

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely. Additional position information

Excellent communication, ability to interact with diverse populations, and professional representation of SCHS. Basic to intermediate computer/EMR/MS Office skills. Strong teamwork and collaboration, ability to multi-task and work independently, attention to detail, strong analytical/problem-solving/decision-making, and organizational skills. Physical requirements

Details include standing, walking, lifting 1-25 pounds frequently; occasional lifting 25-50 pounds; bending, stooping, climbing, reaching; exposure to PPE; BBP exposure risk; schedule: 20 hours weekly; shift: First Shift 08:00-16:30; regular caregiver type; non-exempt; Monday-Friday.
