Position Summary
The Discharge Coordinator is a professional who plans, manages, and coordinates the discharge plan of the acutely hospitalized patient. The discharge Coordinator works in conjunction with the multidisciplinary team and is knowledgeable of available community resources and the healthcare benefits of the acute patient. The discharge Coordinator will have excellent communication skills both written and verbal and be able to work in an intense environment.
What It Takes to be Successful
Previous acute care, healthcare, or related experience
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Must demonstrate customer service skills appropriate to the job
Excellent written and verbal communication skills in English
Ability to maintain a work pace appropriate to the workload
Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications
One year or above acute care hospital discharge planning experience
Working Knowledge of regulatory guidelines including Medicare/CMS guidelines, TJC standards, and NCQA standards
CA RN or LVN license
Knowledge of University and medical center organizations, policies, procedures and forms
