The Care Manager (Designated Care Manager) is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. The Care Manager provides hands‑on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home‑like environment. The role demonstrates Sunrise Senior Living’s mission “to champion quality of life for all seniors” and complies with all applicable laws, regulations, policies, and procedures. Responsibilities

Resident Care

Review, read, notate, and initial Daily Log to document pertinent resident information. Participate in the development and monthly updates of ISPs. Review designated assignments and remain accountable for a designated group of residents during the shift. Observe, report, and document changes in resident condition (skin, behavior, alertness, weight, diet, activity participation). Attend daily Cross‑Over meetings led by the lead care manager. Notify supervisor and resident care director of increased care needs. Respond promptly to security system alerts and resident call bells. Greet guests, families, residents, and team members; answer phone calls courteously. Assist with continence management and ensure proper sanitation of resident suites and community restrooms. Communicate with families as a resource. Promote resident independence, safety, and dignity in accordance with Resident Rights and Sunrise Principles of Service. Assist residents with activities of daily living (ADLs) and encourage participation in activities per the ISP. Engage residents in life‑skill and enrichment activities throughout the day. Maintain a warm, calm, and positive atmosphere. Follow safety regulations, standard care precautions, and hygiene practices. Host and participate in daily resident activities. Risk Management and General Safety

Partner with the community team to ensure compliance with occupational health and safety regulations. Practice safety procedures, including use of PPE, fire extinguishers, SDS, and lockout/tagout. Report all accidents, incidents, unsafe conditions, and hazardous equipment immediately. Keep walkways, cords, carts, and equipment clear of exits and fire hazards. Handle and dispose of blood‑borne pathogens, bodily fluids, infectious waste, sharps, and hazardous materials safely. Report occupational exposures promptly. Store oxygen tanks safely and monitor levels. Housekeeping and Laundry Services

Maintain and clean assigned resident rooms (bed making, trash removal, spot sweeping, mopping). Wash resident laundry as noted in the ISP. Wash and fold dining room linens and napkins. Complete assigned housekeeping tasks and keep common areas tidy. Dining Service

Serve meals in the dining room and assist with set‑up and clean‑up. Promote a pleasant dining experience for residents and guests. Participate in pre‑meal meetings. Follow resident diet orders as indicated on the ISP. Document any changes in resident dining habits. Provide room service delivery as needed. Practice safe food handling in compliance with universal precautions. Specific Responsibilities for the Reminiscence Neighborhood Care Manager

Invite, Encourage, and Assist (IEA) residents in life‑skill and enrichment activities. Encourage residents to attend afternoon social activities. Incorporate “Creating Pleasant Days” into daily routines. Blend multi‑sensory experiences into residents’ days. Participate in monthly letters home and letter‑writing with residents and families. Training and Contributing to Team Success

Participate as a team member and work toward team goals. Demonstrate the Team Member Credo in daily interactions. Commit to serving residents and guests through Sunrise Principles of Service. Engage in employee engagement programs and surveys. Attend regular meetings (Town Hall, Department Team, Cross‑Over, etc.). Complete required training modules and maintain compliance with infection‑control policies. Perform other duties as assigned. Core Competencies

Ability to stay calm and react effectively in difficult situations. Ability to handle multiple priorities. Strong written and verbal communication skills. Organizational and time‑management competence. Good judgment, problem‑solving, and decision‑making skills. Experience and Qualifications

High School diploma/GED (may be required per state/provincial regulations). CPR and First Aid certification as required. Must be at least 18 years of age. Previous experience working with seniors preferred. Desire to serve and care for seniors. Ability to make decisions in the resident’s best interest. Drivers of Sunrise vehicles must sign the driver job description and adhere to safety and regulatory requirements. Benefits
