Job Overview The Care Manager / Designated Care Manager provides the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. The role involves hands‑on care, physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while maintaining a safe and comfortable home‑like environment. Responsibilities
Review, read, note, and initial the Daily Log to document and understand resident information.
Participate in the development and monthly updates of ISPs.
Be responsible for a designated group of residents during the shift: locate residents, conduct physical checks, and monitor their well‑being.
Observe, report, and document changes in symptom or condition (skin, behavior, alertness, weight, diet, activity participation).
Attend daily Cross‑Over meetings with the lead care manager.
Notify supervisor and resident care director of any increased care needs.
Respond promptly to security system and resident call bells, resetting as needed.
Greet guests, family members, residents, and team members with courtesy and professionalism.
Answer and direct phone calls in a timely manner.
Assist with continence management and ensure proper disposal of continence products to maintain sanitation of resident suites and community restrooms.
Communicate with families as a resource when required.
Help residents maintain independence, physical safety, and dignity in accordance with Resident Rights and Sunrise Principles of Service.
Assist residents with activities of daily living (ADLs) and encourage participation in activities as indicated on the ISP.
Engage residents in life‑skills and enrichment activities throughout the day, including reminiscence.
Maintain a warm, positive, calm environment and uphold safety regulations.
Practice standard care precautions for cleanliness, hygiene, and health.
Host and engage in daily resident activities, where appropriate.
Risk Management & Safety
Partner with the community team to ensure compliance with occupational health and safety regulations.
Practice safety procedures (PPE, fire extinguishers, SDS, lockout/tagout).
Report accidents, incidents, unsafe or hazardous conditions immediately.
Keep cords, carts, equipment out of the way and in compliance with fire codes.
Understand and practice safe handling of blood‑borne pathogens, bodily fluids, infectious waste, and hazardous materials.
Ensure safe storage, exchange, and monitoring of oxygen tanks.
Housekeeping & Laundry
Maintain resident rooms: make beds, empty trash, store clothes, spot‑sweep, and mop as needed.
Wash resident laundry per ISP and needs.
Wash and fold dining room linens and napkins.
Complete assigned housekeeping tasks and keep common areas tidy.
Dining Service
Serve meals in the dining room and assist with set‑up and cleanup.
Promote a pleasant dining experience.
Assist with dining room set‑up and clean‑up as assigned.
Participate in pre‑meal meetings.
Follow residents’ diets per ISP and Confidential Diet Board/Chart.
Observe and document any changes in residents’ dining habits.
Provide room‑service delivery as needed.
Practice safe food handling per universal care precautions.
Specific Responsibilities for Reminiscence Neighborhood Care Manager
Encourage residents in life‑skills and life‑enriching activities per ISP and demographic profile.
Encourage residents to attend afternoon social activities.
Integrate the concept of
“Creating Pleasant Days”
into residents’ daily routines.
Blend multi‑sensory experiences into the resident’s day.
Participate in monthly letters home and letter writing with residents and families.
Training and Team Success
Participate as a team member and work toward team goals.
Demonstrate the Team Member Credo in daily interactions.
Commit to serving residents and guests through Principles of Service.
Contribute to engagement programs and processes, including surveys and improvement workshops.
Attend regular meetings: Town Hall, Department Team, Cross‑Over, etc.
Attend training modules with Assited Living Coordinator, Resident Care Coordinator, or Reminiscence Coordinator.
Maintain compliance with infection control techniques and bio‑hazard protocols.
Complete required training per state/organisation standards.
Perform other duties as assigned.

Experience and Qualifications
High School diploma or GED (accepted; may be required by state/provincial regulations).
CPR Certificate and First Aid as required by state/provincial regulations.
Must be at least 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
Ability to make choices and decisions and act in the resident’s best interest.
Drivers of a Sunrise vehicle must sign the Driver job description and understand safety and regulatory compliance requirements.
