Job Summary
In this position the candidate will provide pharmaceutical care to patients admitted to the UK HealthCare Good Samaritan Hospital. Responsibilities include: reviewing and perfecting medication orders, reviewing drug profiles to ensure appropriateness of therapy and contacting and conferring with physicians and other practitioners to optimize medication therapy. Actively participating in all aspects of the medication reconciliation process and promotion of economically conscientious use of medication. Daily activities may include active participation on patient care rounds, review of patient profiles, and provision of pharmacokinetic evaluation of therapeutic regimens. Responsibilities
Review and perfect medication orders Review drug profiles to ensure appropriateness of therapy Confers with physicians and other practitioners to optimize medication therapy Participate in medication reconciliation process Promote economical use of medication Participate in patient care rounds and review patient profiles Provide pharmacokinetic evaluation of therapeutic regimens Qualifications
BS Pharmacy or Pharm D 2 – 3 years of inpatient hospital experience preferred Licensed Pharmacist or License Eligible in Kentucky PGY1 residency preferred; or equivalent 2 – 3 years inpatient hospital experience highly preferred Details
