In this position the candidate will provide pharmaceutical care to patients admitted to the UK HealthCare Good Samaritan Hospital. Responsibilities include: reviewing and perfecting medication orders, reviewing drug profiles to ensure appropriateness of therapy and contacting and conferring with physicians and other practitioners to optimize medication therapy. Actively participating in all aspects of the medication reconciliation process and promotion of economically conscientious use of medication. Daily activities may include active participation on patient care rounds, review of patient profiles, and provision of pharmacokinetic evaluation of therapeutic regimens. Responsibilities

Review and perfect medication orders Review drug profiles to ensure appropriateness of therapy Contact and confer with physicians and other practitioners to optimize medication therapy Actively participate in all aspects of the medication reconciliation process Promote economically conscientious use of medication Participate in patient care rounds and review of patient profiles Provide pharmacokinetic evaluation of therapeutic regimens Skills / Knowledge / Abilities

Knowledge of inpatient hospital pharmacy practice Strong clinical skills Ability to prioritize Qualifications

Required Education : BS Pharmacy or Pharm D Required Related Experience : 2 – 3 years of Inpatient Hospital Experience preferred Required License/Registration/Certification : Licensed Pharmacist or License Eligible in Kentucky Physical Requirements : May require standing or sitting for > 5 hrs at a time Preferred Education/Experience : PGY1 residency preferred; or equivalent 2 – 3 years for Inpatient Hospital Experience highly preferred Deadline To Apply
