This position could work 100% remote if preferred, supporting ages 5+ via video visit. Patients are in their homes or via telemedicine, presenting to a clinic.
Job Summary The Nurse Practitioner (NP) provides services to patients in designated care settings, including acute, short- and long-term care, by assisting physicians, assessing patients, and treating injuries and ailments. Responsibilities include obtaining a thorough medical history, performing examinations or assessments, and developing and establishing appropriate diagnoses. Collaborates with other healthcare professionals to coordinate care, creates treatment plans, and prescribes medications as legally authorized. Maintains detailed patient records, consults with or refers to specialists when needed, and provides patient education to promote health and prevention. May cover hospital consultations, rounding, and assist with procedures. Applies evidence-based practice guidelines and maintains current knowledge of state regulations and payer systems. Demonstrates extensive knowledge of medical principles, practices, and techniques.
Responsibilities
Assess patients, obtain medical histories, perform examinations, and determine patient needs.
Collaborate with providers to develop and implement treatment plans; prescribe medications as authorized.
Coordinate care, round on patients when necessary, and consult with or refer to specialists as needed.
Maintain complete and detailed health care records; ensure timely documentation.
Educate patients and families on self-management, prevention, and treatment plans.
Provide hospital consultations, rounding, and assist with procedures as needed.
Engage in research and integrate evidence-based guidelines into care.
Maintain knowledge of state regulations, reimbursement, and payer systems (Medicare/Medicaid, managed care, private sources).
Qualifications
Master’s, post-master’s, or doctoral degree from an accredited nurse practitioner program (CNE or NLNAC accredited).
Current licensure as an advanced practice nurse in the applicable state; current DEA license to prescribe controlled substances.
Certified Psychiatric & Mental Health Nurse Practitioner (PMHNP-BC).
Demonstrated current clinical competence with an adequate patient volume in the past 12 months (or recent completion of degree/program).
Ability to maintain department-specific competencies and certifications.
