Overview

IHOT Case Manager role at La Familia. The IHOT program specializes in field-based and in-home outreach and engagement to adult individuals with severe mental illness (SMI) who are disengaged from mental health treatment or reluctant to engage. The goal is to refer and link clients to specialty mental health services, such as psychiatry, psychotherapy, and case management services. The In-Home Outreach Team (IHOT) operates to augment and enhance the county’s mental health services by engaging the hardest-to-serve SMI populations. We are seeking an enthusiastic and experienced Case Manager to join the IHOT Program. The Case Manager coordinates services for individuals with SMI who are disengaged from treatment. Responsibilities include the entire case management process: assessment and evaluation, planning, authorizing, advocacy, monitoring, and documentation. This position is covered under the SEIU 1021 Collective Bargaining Agreement. In addition to base pay, negotiated annual increases apply as follows. Essential Job Functions And Responsibilities

Participate in peer-to-peer learning by being available for occasional job shadowing. Perform regular assessments of clients’ needs in housing, daily activities, counseling, coordination of other care, social relationships, family relationships, symptom management, entitlements (e.g., SSI), budgeting, and related areas. Provide direct services including outreach, engagement, referral, linkage, and follow-up at clients’ and families’ preferred locations. Collaborate with Program Supervisor in facilitating weekly team meetings where client cases are reviewed and team updates are provided. Support IHOT team members in planning and conducting crisis interventions. Collaborate with the Lead Clinician in planning and facilitating workshops and support groups for clients and community members. Provide feedback to Program Supervisor about program needs and morale; assist IHOT staff to troubleshoot and solve operational needs and concerns to ensure quality service delivery. Conduct community outreach to homeless clients and those in various institutional settings to establish contact, build rapport, and provide linkage services. Provide field-based services and transportation to appointments and resources as necessary. Meet with clients and caregivers/family members regularly to establish and maintain contact, build rapport, develop care plans, and facilitate healthy dialogue and coping skills. Provide case management duties and connect clients to resources (e.g., General Assistance, SSI, housing referrals, and services within ACBHCS and other community resources). Maintain client confidentiality and HIPAA-compliant charts; complete progress notes and documentation; ensure productivity and client contact hour requirements are met. Attend IHOT staff meetings and trainings; participate in supervision and relevant meetings for professional development. Serve as a liaison with ACBHCS and keep policies and trainings up to date. Comply with all agency policies and procedures. Attend trainings and participate in events relevant to key responsibilities. Track and report on key metrics for responsible function areas. Lead and collaborate on special projects aligned with agency objectives. Qualifications & Experience

BA degree in psychology, counseling, or social work and two years of experience in the mental health field, or four years of experience in the mental health field, or equivalent education/experience. Bilingual Spanish/English preferred. Experience with SMI populations and case management concepts (including individual rehabilitation, family supports, housing, employment, education, and crisis interventions). Demonstrated ability to work with diverse teams and populations. Strong organizational and prioritization skills; ability to multi-task in dynamic settings. Effective writing skills for documenting services and coordinating casework with IHOT staff. Ability to collaborate in a team environment and adapt to feedback constructively. Experience in nonprofit/community settings preferred. Excellent customer service orientation and ability to work with diverse populations. Discretion with confidential information; proficient in MS Office and Google Workspace. Valid CA Driver’s license required.
