Position Overview
The Social Service Director direct and manages the day-to-day operations of the Social Service department in accordance with current federal and state regulations in addition to the community policy.
Key Responsibilities
Supervise the social services department in the day-to-day operations ensuring to set job assignments and set priorities.
Develop goals and objectives for the department; motivate and manage care team members to accomplish the same.
Ensure new care team members are effectively oriented; ensure an ongoing educational program is in place.
Develop positive relationships on behalf of the community with regulatory agencies, residents, and families including participating during family orientation and family council.
Assist in preparing budget for the department; administer approved budget.
Implement policies/procedures with follow-up and supervision of care team members to ensure compliance.
Provide direct psychosocial intervention.
Perform resident assessments at admission, upon condition change, and/or annually.
Create, review, and update care plan and progress notes.
Coordinate resident visits with outside services, dental, optical, etc.
Attend and document resident council meetings.
Assist resident families in coping with skilled nursing placement, physical illness, and disabilities of the resident as well as the grieving process.
Work with the resident, family, and other care team members to plan discharge.
Conduct in-service programs to educate staff regarding psychosocial issues and patient rights.
Qualifications
Bachelor's degree in social work or related field.
Certified as required by the state of employment.
One year of social work experience in a long-term care environment.
One-year supervisory experience in a setting service setting.
