As a Master’s Level Case Manager, you will provide direct support to middle school students (grades 6–8) at Chestnut TAG, Impact Prep, and Springfield Legacy Academy who are at risk for emotional, behavioral, or substance use challenges. You’ll work with students, families, and school staff to improve outcomes, reduce risks, and support academic success. Responsibilities
Maintain a caseload of 15–20 youth and families, providing intensive Wraparound services Conduct comprehensive assessments, including CANS, CRAFFT, SBIRT, and ACES, to identify strengths and needs Develop and update service plans, ensuring family voice and youth engagement Collaborate with Bachelor’s level Case Managers for risk management and safety planning Facilitate psychoeducational groups on social skills, emotional regulation, and risk reduction for substance use Provide parent education and skills training sessions throughout the year Deliver support in school, home, and community settings, including after school hours Consult with teachers, school staff, and community providers to coordinate supports Participate in IEP/504 meetings and contribute to treatment planning Track student outcomes, progress, and barriers while maintaining timely documentation What You’ll Do

Master’s degree in Counseling, Psychology, Social Work, or a related field required CANS-MA certification required (or willingness to obtain upon hire) Experience working directly with youth, families, and substance use issues preferred Strong organizational, communication, and facilitation skills Ability to work with culturally diverse populations Knowledge of school-based and community behavioral health resources Comfortable with technology, including documentation systems, word processing, and email Valid driver’s license and reliable transportation required Details
