Transitional Aged Youth (TAY) – Case Manager / Life Coach Location: On-Site – Fresno or Stockton, CA (TBD). Schedule: Full-Time Non-Exempt. Department: Transitional Age Youth. Reports to: Core Program Director I–TAY. Pay Range: $23-24/Hour. Position Summary Our Stockton office is seeking a Life Coach III to provide case management and supportive housing services for up to 12 young adults participating in licensed supportive housing programs. As a TAY Case Manager/Life Coach with Aspiranet, you will empower young adults (ages 18–25) transitioning from foster care to independent living. You will provide personalized support in housing, employment, education, and wellness to help bridge gaps and foster resilience, self-sufficiency, and long-term success for youth preparing for adulthood. Aspiranet’s Transitional Aged Youth (TAY) services support youth ages 18–25 transitioning from foster care to independent living, with guidance in housing, employment, education, wellness, and other needs for a successful transition. Key Responsibilities – Your Skills and Duties Work collaboratively to engage the Youth and other support team members in identifying strengths and needs to develop and implement a service plan. Provide weekly case contact and support to assess program compliance, regulatory standards, and progress. Conduct assessments and screenings to ensure identified needs and safety issues are addressed via referrals, follow-up, and safety planning. Qualifications Education & Work Experience Bachelor’s degree in a related field, and 2 years of experience working with children and/or transition age youth; knowledge of the foster care system. Licenses & Certifications Meets all state-required conditions of employment as set forth by funder and regulatory body; e.g., Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid CDL and clean driving record.
