Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and Support Staff. Essential Duties and Responsibilities: Provide primary care services in accordance with Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities. Quality/Compliance Review and sign MARs as requested by facilities, sign all orders specific for Primary Care at the facility or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review and address labs within 24 hours. Manage narcotic prescriptions for your facility’s patients per Eventus WholeHealth Narcotic Rx Policy. Allocate sufficient time during visits for administrative paperwork. Complete documentation and submit billing records according to policy. Provide the facility with a hard copy of documentation within 48 hours for inclusion in the patient’s medical record. Maintain open, positive communication with facility staff and administration. Provide verbal or written feedback on medications and other caregiver information. Meet with the facility care coordinator or administrator before starting and before leaving each day. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical standards governing healthcare delivery. Maintain patient confidentiality at all times, including appropriate use of phones, emails, texts, charts, and EMR, in compliance with HIPAA regulations. Consult with the clinical supervisor before refusing any resident or family request concerning resident care. Qualifications/Education: Master’s degree in Nursing from an accredited institution and a valid state license to practice medicine. Current certifications as required. OR Graduation from an accredited Physician Assistant program, passing the national certification exam, and licensure as a Physician Assistant in your state. Geriatric, family, or psychiatry specialization preferred.
