Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff. Essential Duties and Responsibilities:

Provide primary-care services according to Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities. Quality/Compliance: Review and sign MARs as requested by facilities, sign all orders specific to Primary Care at the facility or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review and address labs within 24 hours of receipt. Manage narcotic prescriptions for facility patients as per Eventus WholeHealth Narcotic Rx Policy. Allocate time during visits for administrative paperwork. Provide on-call triage duties as per protocol, adhering to the latest triage schedule. Complete documentation and submit billing records per policies, providing a hard copy within 48 hours. Maintain open communication with facility staff and administration, providing verbal or written instructions and feedback. Meet with facility administrators or care coordinators before and after patient visits. Adhere to Eventus WholeHealth standards, policies, legal, regulatory, and ethical guidelines, including HIPAA confidentiality. Consult with the clinical supervisor before refusing resident, family, or facility requests regarding care. Qualifications/Education:

Master's degree in Nursing from an accredited institution and current, valid license to practice in your state. OR graduation from an accredited Physician Assistant program, passing the national certification, and licensure in your state. Geriatric, family, or psychiatry specialization preferred.
