Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers, including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.
Essential Duties and Responsibilities:
Provide primary-care services following Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, or internal medicine related to long-term care facilities. Quality/Compliance
Review and sign MARs as requested by facilities, sign all orders specific for Primary Care within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review and address labs within 24 hours. Manage narcotic prescriptions specific to facility patients per Eventus WholeHealth Narcotic Rx Policy. Allocate sufficient time during visits for administrative paperwork. Complete documentation and submit billing records according to policies. Provide the facility with a hard copy of documentation within 48 hours for inclusion in the patient’s medical record. Maintain open communication with facility staff and administration. Provide verbal and written feedback regarding medications and caregiver information. Meet with the facility care coordinator at the start and end of each day. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical guidelines. Ensure compliance with healthcare regulations. Maintain patient confidentiality at all times, including proper use of phones, emails, charts, and EMR. Follow HIPAA rules and regulations. Consult with the clinical supervisor before refusing care requests from residents, families, or facility staff. Qualifications/Education:
Masters degree in Nursing from an accredited institution and a valid state license to practice medicine. All necessary certifications must be current. OR Graduation from an accredited Physician Assistant program, passing the national certification exam, and licensure in your state. Geriatric, family, or psychiatry specialization is preferred.
