Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.
Essential Duties and Responsibilities:
Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities.
Quality/Compliance
Review and sign MARs as requested by facilities, sign all orders specific to Primary Care within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review and address labs within 24 hours of receipt. Manage narcotic prescriptions for your facility’s patients according to Eventus WholeHealth Narcotic Rx Policy. Allocate sufficient time during visits for administrative paperwork.
Perform on-call triage duties as per the triage protocol policy. Adhere to the scheduled triage policies, which may be updated based on a majority vote of involved providers.
Complete documentation and submit billing records per policies. Provide the facility with a hard copy of documentation within 48 hours for inclusion in the patient’s medical record.
Maintain open communication with facility staff and administration. Provide verbal or written feedback regarding medications and caregiver information. Meet with the RCC or care coordinator before and after patient visits each day.
Adhere to Eventus WholeHealth standards, policies, procedures, and all legal, regulatory, and ethical guidelines governing healthcare delivery.
Maintain patient confidentiality at all times, including proper use of communication tools and electronic records, in compliance with HIPAA regulations.
Consult with the clinical supervisor before refusing a resident or family request regarding resident care.
Qualifications/Education:
Master's degree in Nursing from an accredited institution and a valid state license to practice medicine. All necessary certifications must be current.
OR
Graduation from an accredited Physician Assistant program, passing the national certification exam, and licensure as a Physician Assistant in your state. Geriatric, family, or psychiatric specialization is preferred.
