program GENERAL FUNCTION
Assess, plan and coordinate needed support and services to children who have physical, cognitive, and emotional disabilities. Service Coordinators also support immediate family members and help problem solve issues/concerns. This position has a significant impact on the children and families served, and on the organization, by helping children receive items and services they need to stay within their home and community (least restrictive and most natural environment) rather than being placed in more restrictive environments. SUPERVISES
No supervisory responsibilities. RESPONSIBILITIES — ESSENTIAL FUNCTIONS
Conduct assessments and develop and implement Individual Service Plans for children in the Children’s Community Options Program (CCOP) and the Children’s Long-Term Support (CLTS) Medicaid Waivers Program. Provide direct supportive services to children and families on caseload including information/referral, supportive counseling, and assistance in applying for financial assistance programs. Conduct home visits and/or virtual visits to discuss family issues/concerns and document all relevant information within CCOP and CLTS Medicaid Waivers programs. Complete Functional Screens (initial, annual, updates) and update Children’s Functional Screens annually. Complete paperwork other than Functional Screens (case notes, letters, emails, faxes, funding requests/authorizations, WPS authorizations, ISPs, outcomes, referrals, reviewing medical records, IEPs, parental requests for items/services, incident reports through Citr, data entry into the ForwardHealth Portal using Eligibility and Enrollment Streamlining (EES), high-cost funding requests, parental fee calculations, and other state/county forms and assessments). Make home visits to conduct intakes, reviews, check-ins, provide support, attend school meetings and/or medical appointments. Make telephone calls to clients, providers, state contacts, therapists, evaluators, contractors, and other professionals involved with County-operated services. Create and help manage CLTS Waiver Third-Party Administration (TPA) authorizations with the Business Office staff. Perform duties in compliance with the Medicaid Home and Community-Based Services (HCBS) Waiver Manual for the CLTS Waiver Program and standards in Medicaid Targeted Case Management regulations. Manage expenditures of CCOP funds, submit timely vouchers, and work with the Business Office on State-required Reporting Systems (Human Services Reporting System and Program Participation System). Know and practice Wood County safety policies and perform all tasks safely. OTHER DUTIES
Research items and services that clients need. Follow established Wood County Human Services Department policies and procedures, and comply with federal and state policies and standards. Participate in unit meetings and attend required training/conferences. Ensure proper case files are maintained in compliance with Department policy and applicable regulations. Monitor service costs for clients in CLTS and CCOP. Exercise other duties as assigned. MINIMUM TRAINING AND EXPERIENCE
1.
A Bachelor of Arts/Bachelor of Science in a health or human services related field and at least one (1) year of professional experience working with individuals and/or children with disabilities required. Knowledge of children with DD (Developmental Disabilities), SED (Severe Emotional Disturbances), and PD (Physical Disabilities). 2.
Knowledge of the service delivery system and available integrated services or the need to develop them. 3.
Must complete the approved DHS waiver basics training course and the Mandated Reporter Online Training. Until training is completed, the Coordinator must work under supervision. 4.
Must become certified to perform the Children’s Long Term Support Functional Screen within one (1) month of employment. Candidate must possess basic living skills, the ability to understand and follow directions, the ability to read and write for reports; and the ability to interview, counsel and communicate. Travel to off-campus sites is required. A valid driver’s license and a licensed/insured automobile or other reliable transportation is required. Basic office equipment required: computer/tablet, printers, telephone, copy machine, fax, calculator; familiarity with Microsoft Word and Excel and Streaming Healthcare Solutions software.
