is currently seeking an experienced
Weekend Physical Therapist (PT)
for home health visits throughout Berks County. This office services adult and geriatric clients on a per-visit basis. Requirements include at least one year of clinical experience as a licensed Physical Therapist. You will be an essential part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home. video . Responsibilities: Make home visits to clients in designated areas. Perform diagnostic tests, including mobility, strength, balance, and safety assessments. Implement wound care interventions using evidence-based dressings and topical agents. Develop and update individualized care plans with therapeutic exercises, gait training, and other interventions. Assess and revise care plans, collaborating with nursing staff as needed. Educate clients and families on rehabilitative activities to promote health and independence. Document client observations and interventions accurately using electronic tablets. Qualifications: Current Pennsylvania Physical Therapist (PT) license. Minimum one year of recent PT experience. Graduation from an APTA-approved program. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills.
