The Nurse Navigator coordinates all services required to provide multidisciplinary, quality-assured, appropriate case selection and cost-effective care to cardiovascular and electrophysiology patients. The Nurse Navigator serves as a resource for patients, physicians and others, acting as educator, quality improvement contributor and consultant to patients referred. Makes initial contact with the patient/family to establish a professional relationship and understanding of the diagnosis, treatment and follow-up regimen. Assesses the patient’s status and works with physicians to determine an appropriate plan for the patient, ensuring adherence to regulatory and vendor guidelines. Develops, manages and redesigns, as needed, the process for scheduling a procedure through discharge for patients in the CV department. Actively participates in outstanding customer service and maintains respectful relationships with all stakeholders. The value you’ll bring to the team

Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner. Facilitates timely scheduling with coordinators, supervisors, physicians, and patients. Develops, plans, and implements evidence-based measures to improve the quality of care for electrophysiology and cardiovascular patients, ensuring compliance with regulatory guidelines. Supports and assists with data collection for process and quality improvement, understanding department performance trends and standards. Qualifications

The expertise and experience you’ll need to succeed : Bachelor's of Nursing Required 3+ years Work Experience Required Registered Nurse (RN) Required Basic Cardiac Life Support (BCLS) Required Advanced Cardiac Life Support Cert (ACLS) Required
