We are looking for a kind, compassionate Practice Nurse to join our forward thinking, tight-knit team. Working in general practice will enable you to buildmeaningful relationships with patients, play a vital role in a practice teamalongside a range of health professionals, and give more personal care topatients. We are offering 16-20 hours per week, with scope to increase overtime as we continue to grow (or to become a job-share). Our operating hours are Monday to Friday 08:00-18:30. We need cover on Fridays and are happy to be flexible on other days. Main duties of the job

The role of thepractice nurseis to be responsible for the deliveryof practice nursing services, working as part of the practice multidisciplinaryteam, delivering care within their scope of practice to the patient population. The practice nurse will be responsible for a number of clinical areas such ashealth promotion, long-term condition management, health prevention,vaccination, well women and well man clinics, as well as supporting themanagement team in the reviewing of clinical policy and procedure. The successful candidate will be highlyorganised, flexible, and able to prioritise effectively. They will be highlyobservant, able to assess patients and take responsibility for determining thebest course of action. As with all nursing roles, communication skills, problemsolving, and good judgement are crucial. Previous General Practice / primary care experience would be helpful but is not a requirement. Thepost holder will work as part of a small clinical team consisting of three GPs, two nurses, two healthcare assistants and team of support staff. They will be responsible for sharing clinical admin duties and checks and there is a wide range of training opportunities available to help maintain competencies and develop new skills and areas of specialism. Primary responsibilities include: Identify,manage, and support patients at risk of developing long-term conditions,preventing adverse effects on the patients health Provideroutine nursing care to patients as required in accordance with clinical basedevidence (e.g. NICE) Providevaccination and travel medicine services Providewound care (ulcer, compression bandaging, Doppler etc.) to patients Requestpathology services as necessary Processpathology results as required Providelong-term condition clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as required Maintainaccurate clinical records Ensureread codes are used effectively Assessand treat minor illness Provideand implement well woman clinics and well man clinics Provideand implement vaccination programmes Prioritisehealth issues and intervene appropriately Recognise,assess, and refer patients presenting with mental health needs Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice) Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies Deliveropportunistic health promotion where appropriate Maintainbaby immunisation records Maintainsmears results logs Actas the Practices Infection Prevention and Control Lead Manageemergency drug kit and resuscitation kit Supportingthe Practice to achieve its annual performance targets (e.g., QOF) through therecall, review, and assessment of patients Person Specification

Qualifications

- Nursing degree or equivalent experience - Registered Nurse (NMC) - Evidence of immunisation training and assessment of competence in last 2 years - Trained and registered to take smears (or working towards this) - Diploma(s) in long-term condition(s) such as Asthma, COPD, Diabetes, Coronary heart disease Knowledge and Skills

- Ability to record accurate clinical notes - Ability to work within own scope of practice and understanding when to refer to senior clinicians - Understanding of the importance of evidence-based practice - Understanding of the requirement for PGDs and associated policy - Competent in the use of patient record management systems (e.g., SystmOne) - Ability to follow clinical policy and procedure - Immunisations - Pathology tests and processing the results, advising patients accordingly ECGs - Wound care / minor injuries - Review of long-term conditions Experience

- Experience of working within hospital, primary care, or community service setting (either as a qualified RGN or as part of RGN training) - Long-term condition management - 2+ years working in a primary care environment in the last 5 years Disclosure and Barring Service Check
