has a full-time opportunity for an experienced

Physical Therapist (PT)

performing

home health visits

for our senior living office. This office services adult and geriatric clients on a per-visit basis in senior living facilities throughout Guilford County and Asheboro, North Carolina.

Applicants must have at least one year of prior clinical experience as a licensed Physical Therapist. As a home health Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities for a PT - Physical Therapist include:

Make home visits to clients in designated geographic areas.

Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.

Implement protective interventions to prevent wounds and promote wound healing using evidence-based dressings and topical agents.

Develop and execute individualized care plans, incorporating manual therapeutic exercises, gait training, and balance interventions.

Regularly assess and update the care plan, collaborating with nursing staff within the multidisciplinary team as needed.

Educate clients, family members, or representatives on rehabilitative activities to promote health, safety, and independence.

Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

Current North Carolina Physical Therapist (PT) license.

At least one year of recent work experience as a PT.

Graduation from an American Physical Therapy Association-approved program or equivalent, evidenced by transcript or diploma.

Ability to work independently and manage time effectively.

Strong interpersonal, organizational, and problem-solving skills.

Proficiency in computer skills; experience with electronic medical records (EMR) preferred.
