Physical Therapist (PT)
for a
full-time
opportunity performing
home health
visits for our Anne Arundel office. This office services adult and geriatric clients on a per-visit basis in the
Southern Anne Arundel County
area. Applicants must have at least one year of clinical experience as a licensed Physical Therapist. As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home. Performing diagnostic tests and measurements such as joint mobility, transfer status, ambulation patterns, muscle strength and endurance, balance, and safety assessments. Implementing protective interventions to prevent wounds and promote healing, utilizing evidence-based dressings and topical agents. Developing and executing individualized care plans, including manual therapeutic exercises, gait training, and balance interventions. Regularly assessing and updating the care plan, collaborating with nursing staff as needed. Educating clients and their families on rehabilitative activities to promote health, safety, and independence. Documenting all observations, interventions, and evaluations accurately using a state-of-the-art tablet. Qualifications: Current Maryland Physical Therapist (PT) license. Minimum of one year recent clinical experience as a PT. Graduation from an American Physical Therapy Association (APTA) accredited program, evidenced by diploma or transcript. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Proficiency with computers; experience with electronic medical records (EMR) preferred.
