Base pay range $120,000.00/yr - $135,000.00/yr

Part Time Physical Therapist Case Manager – Coverage Area: Santa Cruz County In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively.
What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
Help patients with mobility, movement, and regain their independence.
Create detailed treatment and care plans for each patient.
Communicate and coordinate with patients’ physicians to ensure quality level of care.
Evaluate and report patients’ progress to MD.

To be a success as a Physical Therapist
Current unrestricted CA Physical Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Knowledge of individual development and human behavior related to illness/injury and cultural influences on health care.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience with home health care (highly preferred).
Experience with high acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case Management fundamentals (highly desired).
Self-Directed.
Empathetic.
Critical thinker.
Outstanding interpersonal skills.
Leadership skills.
Organized.

Wage range: Physical Therapist, Level 1: 120,000.00 – 135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match). Most PT Case Managers are hired as PTIs. Once an PTI can supervise 2 PTAs, take a larger caseload and assume higher decision making and autonomy, they are promoted to PTII with a minimum salary of 135,000 plus 20K bonus (paid quarterly).
