Base pay range
$120,000.00/yr - $135,000.00/yr

Full Time Physical Therapist Case Manager
In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach maximum performance within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Other responsibilities.
What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
Help patients with mobility, movement, and regain independence.
Create detailed treatment and care plans for each patient.
Communicate and coordinate with patients’ physicians to ensure a quality level of care.
Evaluate and report patients’ progress to MD.

To be a success as a Physical Therapist
Current unrestricted CA Physical Therapist License
Current BLS/CPR Certification
Valid CA Driver's License
Knowledge of local community resources
Understanding of how illness/injury and culture affect health care
Knowledge of homecare regulatory guidelines preferred
Experience in home health care highly preferred
Experience with high acuity patients a plus
Experience working with an interdisciplinary team highly desired
Case Management fundamentals highly desired
Self-directed, empathetic, critical thinker, outstanding interpersonal skills
Leadership and organizational skills

Wage range: Physical Therapist, Level 1: 120,000.00-135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers benefits and 401k with up to 4% match. The majority are hired as PTIs; progression to PTII with higher salary and bonuses available.
