Job Summary This role will work 7 on, 7 off shifts, 10 hour shifts from 11 AM to 9:30 PM.

Required Experience and Education

Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree

1 year of pharmacy experience

Eligible for licensure by Texas State Board of Pharmacy (TSBP); Texas license required within 30 days of hire

Sterile compounding (IV) certification accredited by ACPE (upon hire)

Basic Life Support (BLS) certification accredited by AHA or American Red Cross (ARC) (upon hire)

Advanced Cardiac Life Support (ACLS) certification accredited by AHA or ARC required for inpatient positions within 180 days

Preferred Experience

1 year of pharmacy experience

Job Duties

Perform all computer order entry accurately and in a timely manner; review and update medication profiles and patient charges

Review medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety; seek clarification if necessary and document interventions (e.g., ADR)

Work independently with minimal supervision; act as a leader and decision maker

Enforce the formulary system; follow departmental policy and procedure when acquiring non‑formulary products; participate in cost‑effective use and selection of pharmaceuticals

Communicate drug information verbally and in writing to physicians, nurses, and patients; assist in patient/family education and counseling regarding drug therapy and side effects; document clinical activities and demonstrate cost savings

Manage technicians and other supportive personnel; delegate tasks; validate accuracy of medications intended for distribution; audit and sign out controlled substances per state/federal guidelines

Participate in educational programs, in‑services, meetings, orientation and training; take on additional projects as needed

Support P&T initiatives; monitor target drugs for protocol discontinuation, substitution, IV to PO, etc.; collect data, communicate opportunities for improvement; perform therapeutic monitoring and recommend dosage adjustments based on renal function and PK calculations

Maintain records, conduct continuous quality improvement, and perform other duties as assigned
