Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations. Performs all computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (e.g., ADR). Multi-tasks, problem solves independently with minimal supervision, and provides direction when necessary. Maintains competency across multiple areas. Enforces the formulary system and follows policy when acquisition of non-formulary products is necessary. Participates in cost-effective use and selection of pharmaceuticals. Communicates drug information to physicians, nurses, and others in a courteous and professional manner. Assists in patient/family education and counseling regarding drug therapy and side effects. Documents clinical activities with emphasis on cost savings and clinically relevant information for follow-up by colleagues. Manages technicians and other support personnel. Delegates tasks and validates accuracy of medications from IV lab, central pharmacy, or automated dispensing machines. Audits and signs out controlled substances per guidelines and investigates discrepancies. Participates in clinical programs and supports P&T initiatives by monitoring target drugs, collecting data, and communicating opportunities for improvement. Performs therapeutic monitoring and dosage adjustments based on renal function and pharmacokinetics. Duties may include direct patient care, handling patient records, and related responsibilities as required. Performs other duties as assigned. Qualifications

Education: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD). Experience: 1 year of pharmacy experience. English proficiency (speaking, reading, writing). Licenses/Certifications: Eligible for licensure by Texas State Board of Pharmacy (TSBP) on hire and Texas license with TSBP within 30 days (ACPE-IV sterile compounding; IV) and Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS) may be required for inpatient positions within 180 days. Preferred: 1 year of pharmacy experience. Knowledge, Skills and Abilities

Basic computer skills and data entry; English proficiency; good health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting up to 25 lbs, reaching, bending, crouching, etc. OSHA Category III. Physical Demands / Working Conditions

Clinical setting with typical physical demands including standing, walking, and occasional lifting. Job Duties

Coordinate with team, maintain formulary compliance, and contribute to cost-effective medication use. Educate patients and caregivers on drug therapy and side effects; document clinical activities clearly. Lead and supervise technicians; ensure accurate dispensing and submittal of controlled substances per guidelines.
