Job Description Under general supervision, the 2587 Health Worker 3 performs a wide variety of the more difficult paraprofessional duties in a service program of the Department of Public Health. The role works with professional staff in extending effective services to clients of the program served; plans, develops, and follows through on all contacts and cases; and may supervise a small staff of workers, primarily Health Worker I and II. Essential duties include:
Assist in planning recreational, educational, and work therapy activities in therapeutic rehabilitation programs.
Interview and screen patients, identify general conditions, and assist in assessment and treatment planning with professional staff.
Represent program staff in meetings with local community groups, governmental and social agencies.
Conduct craft, recreation, and other activity groups; assist in conducting therapy sessions under professional supervision.
Maintain patient charts and related records; conduct surveys and operate technical equipment.
Supervise others, primarily lower Health Worker classifications.
Plan, implement, supervise, coordinate, publicize, evaluate, and document specialized activities program for patients.
Qualifications
Current Permanent Civil Service City Employee in class 2586 Health Worker 2 who passed probation.
Experience: Two (2) years of verifiable experience within the last five (5) years working with a culturally diverse population, performing at least two of the following duties:
Serving as a liaison between targeted communities and healthcare agencies.
Providing culturally appropriate health education, information, and outreach to targeted populations.
Providing referral and follow-up services or coordinating care.
Providing informal counseling, social support, and advocacy to targeted populations.
Escorting and transporting clients.
Providing courier/dispatcher functions.
Performing pre-clinical examinations of vital statistics (e.g., measuring weight, height, temperature, blood pressure).
Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for six (6) months of required experience.
Selection Procedures Applicants must fully complete the job application. The Training and Experience Assessment (100% weight) is mandatory and will be used to determine eligibility. Qualified applicants are ranked on an eligible list based on additional qualifying points. Additional selection processes may be conducted by the hiring department prior to final hiring decisions.
