South Texas Health System . Position Summary: The Assessment and Referral Coordinator receives referrals and establishes assessment and admission services for patients with behavioral health needs. The objective is to provide an appropriate disposition utilizing internal or external services. The Coordinator ensures that patient information is obtained accurately and conveyed to the necessary entities. Responsibilities: Receive referrals and coordinate assessment and admission services for behavioral health patients. Ensure accurate collection and communication of patient information. Provide appropriate dispositions utilizing available resources. Qualifications: Prior experience with crisis intervention, assessment, and referral processes. Knowledge of psychiatric diagnoses. Commitment to compliance with STHS’s policies, Code of Conduct, and training requirements. Education / Licensure: RN licensed in Texas, or in a compact state, or holding a valid temporary RN license for Texas. ADN or BSN preferred.
