Overview
Care Manager (Designated Care Manager) role at Sunrise of San Rafael. Provides the highest degree of quality care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. Delivers hands-on care and emotional support as outlined in each resident's ISP, while maintaining a safe, home-like environment. Promotes Sunrise's mission to champion quality of life for all seniors in compliance with applicable laws, regulations, and Sunrise standards and procedures. Responsibilities
Resident Care: document and learn about residents via the Daily Log; participate in ISP development and updates; review designated assignments; supervise a designated group of residents during the shift; observe and document changes in residents" condition; attend daily Cross Over meetings; notify supervisor of increased care needs or changes in condition; respond to call bells; greet guests and residents; assist with continence management; communicate with families; support residents in activities of daily living and life enrichment activities; promote independence and safety; maintain warmth and a calm environment; follow safety regulations and standard care precautions; host and engage in daily activities. Risk Management and General Safety: ensure compliance with occupational health and safety regulations; use PPE, fire safety equipment, SDS, and Lockout/Tagout procedures; report accidents and unsafe conditions; keep hazards clear and compliant with fire codes; manage exposure risks and safe disposal of hazardous materials. Housekeeping and Laundry: maintain resident rooms and common areas; handle resident laundry and dining linens; complete assigned housekeeping tasks. Dining Service: serve meals, promote a pleasant dining experience, assist with setup and cleanup, participate in pre-meal meetings, follow ISP diets, document changes in dining habits, and provide room service as needed; practice safe food handling. Reminiscence Neighborhood Responsibilities: support residents with life skills and enrichment activities; participate in monthly letters home; integrate life skills into daily routines and create pleasant days; blend multisensory experiences into daily activities. Training and Team Contribution: participate as a team member, uphold Team Member Credo and Sunrise Principles of Service; contribute to engagement programs and surveys; attendRegular meetings and trainings; comply with infection control policies; maintain required trainings; perform other duties as assigned. Core Competencies: react calmly under pressure; manage multiple priorities; effective written and verbal communication; strong organizational and time management; good judgment and problem solving. Qualifications
High School diploma/GED required or as dictated by state/provincial regulations. CPR and First Aid as required by regulations. Must be at least 18 years old. Previous experience with seniors preferred. Desire to serve and care for seniors; ability to act in residents’ best interests. If applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand safety duties. About Sunrise
