Overview

Care Manager role at Sunrise Senior Living – Sunrise of San Rafael. The Care Manager provides the highest quality of care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. The role includes hands-on care and support as outlined in each resident's ISP, while maintaining a safe, home-like environment and upholding Sunrise’s Mission to champion quality of life for all seniors. Responsibilities

The following responsibilities reflect the essential functions associated with this role: Resident Care: review, read, annotate, and initial Daily Log; participate in ISP development and monthly updates; review designated assignments; be responsible for a designated group of residents during the shift with regular checks; observe, report, and document changes in resident condition (skin, behavior, alertness, weight, diet, activity participation); attend daily CrossOver meetings; notify supervisor of increased care needs and changes in condition; respond promptly to security bells; greet guests, family members, residents, and team members; assist with continence management and sanitation; communicate with families as needed; support residents to maintain independence and safety; lead/participate in ADLs and Encourage/Assist (IEA) per ISP; engage residents in life skills and reminiscence activities; understand resident communication patterns and respond with empathy; maintain warmth and a calm environment; follow safety regulations; practice Standard Care Precautions; host and engage in daily activities. Risk Management & General Safety: ensure compliance with occupational health and safety regulations; practice safety procedures including PPE, fire safety, SDS, and lockout/tagout; report all accidents/incidents and unsafe conditions; keep cords/equipment out of walkways and in line with fire codes; handle exposure to blood/bodily fluids and hazardous materials safely; secure oxygen tanks and monitor prescribed levels. Housekeeping & Laundry: maintain and clean resident rooms; launder resident textiles; manage dining room linens; complete assigned housekeeping tasks; keep common areas clean and tidy. Dining Service: serve and assist in dining rooms; promote a pleasant dining experience; assist with setup/cleanup and attend pre-meal meetings; follow ISP-directed diets and document changes in dining habits; respond to dining needs while upholding Sunrise hospitality and service standards; provide room service when needed; practice safe food handling. Reminiscence Neighborhood Care Manager (Specific Responsibilities): encourage life skills engagement per ISP; support attendance at social activities; integrate “Creating Pleasant Days” and individual life skills into daily routines; incorporate multisensory experiences; participate in monthly letters home with residents and families. Training & Team Contributions: participate as a team member; uphold Team Member Credo and Sunrise Principles of Service; contribute to engagement programs and attend team, Town Hall, and department meetings; complete required training; follow infection control guidelines and state/provincial regulations; perform other duties as assigned. Core Competencies: maintain composure in difficult situations; manage multiple priorities; strong written and verbal communication; organizational and time-management skills; sound judgment and problem solving. Qualifications

High School diploma/GED (as required by state/provincial regulations). CPR and First Aid as required by state/provincial regulations. Must be at least 18 years of age. Previous experience with seniors preferred; desire to serve and care for seniors. Ability to make choices in the resident’s best interest; for employees driving Sunrise vehicles, sign the Driver job description and acknowledge safety duties. Benefits & Compensation
