Practitioner is a medical doctor with fellowship training in infectious disease who provides high-quality medical evaluation, diagnosis, and treatment to patients in a high-volume outpatient, federally qualified health center setting for complex HIV management, infectious disease specialty care, and primary care. This individual manages patients in accordance with standards of care and approved organizational privileges. The incumbent collaborates with other health care providers, social workers, and other professionals in an integrated, evidence-based care team model. Assess and redirect emergent conditions through available emergency services consistent with organization policy. Provide after-hours consultative call on a shared, rotating basis with after-hours triage service, as per organizational protocols. Responsibilities
Provide high quality patient care to all CrescentCare patients in an outpatient clinical setting. Achieve a minimum of 1850 completed patient visits per calendar year (approx. 154 per month for a full-time FTE), subject to change based on organizational needs. Collaborate with care team members to deliver high-quality, patient-centered care and maintain high patient satisfaction. Close notes within 48 hours of the visit; complete all notes no later than 5 business days after the end of the calendar month in which the service was provided. Operate within the scope of practice as outlined by CrescentCare privileges and this job description. Achieve quality metric goals as defined by medical leadership and the quality team. Care Team
Practice at the top of the practitioner’s license and collaborate with care team members to provide high-quality, patient-centered care. Lead or participate in pre-visit planning huddles using data and reporting systems. Communicate effectively with medical and operational staff about patient care issues, using evidence-based practices. Promote teamwork and positive interpersonal relations among staff. Actively participate in team meetings. Provide specialty-specific consultation upon request within the specialty scope of practice. Supervise clinical care, collaborate, and provide clinical consultation to PAs/APRNs as necessary. Qualifications
Education
Medical Degree (MD/DO). Work Experience
MD/DO with successful completion of an ACGME or AOA-accredited residency/fellowship in internal medicine, pediatrics, family medicine, or other CrescentCare-approved specialty as an FQHC. MD/DO with successful completion of an ACGME or AOA-accredited fellowship in infectious disease. Other (Licensure, Certifications)
Louisiana Medical License without restriction. Board certification in the practitioner’s terminal board. Admitting privileges with a hospital directly or a collaborative agreement with a physician who has admitting privileges at a local hospital. Basic Life Support (BLS) Certification. Essential Physical Requirements
Extended periods of sitting; work is primarily office-based. Some travel may be required. May lift up to 25 pounds with equipment and supplies. Total Compensation Package
