Salary Range: $140,000.00/year - $160,000.00/year ; MSN: $140,000.00-$155,000.00; DNP: $158,000.00-$160,000.00. Additional ACT differential pay: $5/hour ($10,400 annually). Responsibilities

Serves as an active member of the site management team, collaborating with the Lead Nurse on the delivery of clinical care. Conducts home and community visits as well as in-clinic care for ACT members. Provides time-unlimited, community-based services to an assigned caseload with a focus on maintaining a stable clinical environment. Takes responsibility for the treatment services for members; coordinates care with ACT team members and other providers to ensure ongoing treatment in the community. Works with informal support systems to provide clinical skills training to increase informal supports (family, landlords, employers, etc.). Maintains high intensity of services and high-frequency contact with members as needed. Supports engagement mechanisms to re-engage members in prescribed treatment when necessary. Assesses need for ACT services and transitions eligible members to less intensive services as appropriate, ensuring continuity of care. Provides crisis services for assigned members, coordinating hospital admissions and discharge planning. Participates in daily team meetings, provides updates on members, and identifies risks and needs to coordinate access to services. Complies with program requirements per SAMHSA guidelines, WICHE, Mercy Maricopa, and others identified by Terros, Inc. Delivers services in accordance with policies, procedures, and clinical protocols; supports counseling skill development and other services as needed. Collaborates on the development and implementation of individual service plans and crisis plans. Connects members and families to additional services and entitlements, identifying funding where applicable. Maintains primary contact responsibilities for a percentage of team members, ensuring accurate documentation and weekly scheduling in the medical record. Documents all member events in a timely manner per policies and protocols. Provides psychopharmacological treatment aligned with the member’s treatment plan and conducts monthly assessments of symptoms and medication responses. Delivers brief therapy and provides education on medications to consumers using shared decision-making. Monitors non-psychiatric medical conditions and medications; communicates with inpatient providers to ensure continuity of care when members are hospitalized. Conducts home and community visits as required by the ACT team. Collaborates with the team leader to monitor treatment and service delivery; educates non-medical staff on medications and health conditions.
