Health and Housing Specialist – Phoenix, AZ
Terros Health is hiring a Health and Housing Specialist to provide direct face‑to‑face community‑based services to adults with substance‑using, serious mental illness, dual diagnosis, or other referred individuals at its Desert Cove location in Phoenix, AZ.
Base Pay Range
$17.18 – $19.21 /hr plus a $3 / hr differential pay.
Responsibilities
Conduct screenings, intakes, and assessments to evaluate clients’ presenting problems and develop treatment plan goals.
Assist clients to obtain and maintain housing, including applying for housing vouchers and identifying housing options for pre‑tenancy (3–6 months) and tenancy (3–18 months).
Provide transportation to clients as medically necessary, using personal vehicle or Terros Health vehicle.
Coordinate care and communicate clinical information with family, behavioral providers, general medical providers, community resources, and other involved supports.
Connect clients to physical health services, including primary care services and medical appointment follow‑up.
Provide excellent customer service to clients, internal and external provider agencies, state entities, and other stakeholders.
Qualifications
GED/High School Diploma with a minimum of 3 years of behavioral health work experience, OR an associate’s degree in any field with a minimum of 2 years of behavioral health experience.
Must have reliable transportation.
Case management experience is a plus.
Housing experience is a plus.
Experience with SMI, dual diagnosis, or substance‑use treatment is preferred.
