Trans and Non-binary Supportive Housing Case Manager . The Case Manager will work in CAP’s Housing & Supportive Services Department to provide services to a caseload of 15 individuals who identify as transgender and/or nonbinary, regardless of HIV status, identified through Coordinated Access, who are experiencing homelessness and may have co-occurring mental health and substance use issues. This is a Full-Time (32 hours/week) hybrid role based in Portland, OR, with travel throughout the five-county service area required. The schedule is Monday-Thursday, 8:30am-5:00pm, with at least 3 in-office days per week. Starting pay is $48,547.20 per year, with increases by tenure up to $56,555. This is a union represented position; compensation, benefits and conditions of work are collectively bargained. What You’ll Do

Provide housing placement, supportive case management, and eviction prevention for clients who are homeless or at risk of homelessness, using a strengths-based model that promotes client self-determination and independence. Collaborate with medical case managers, medical providers, and other care teams to provide coordinated, comprehensive care focused on housing stability for Trans and Non-Binary participants. Establish professional relationships with landlords to create new housing resources and share information with colleagues. Work collaboratively with CAP staff, volunteers, community partners, and clients; communicate effectively across teams. Attend trainings and educational opportunities to remain current on housing, HIV, case management, and related issues. Other duties as assigned. Eligible candidates should have one year of relevant experience or a bachelor’s degree in a human/social services field, prior case management experience, and the ability to travel across the service area. A valid Driver’s License and access to a reliable vehicle are required. Availability for occasional evenings and weekends for CAP events is preferred. Qualifications

One year of relevant experience or a bachelor’s degree in human/social services (e.g., social work, public or community health, psychology) or related field. Prior experience providing case management or similar services. Experience working with ethnically, racially, economically, and sexually diverse populations and people experiencing homelessness, mental health, and/or substance use issues. Ability to travel throughout the service area on a frequent basis. Valid Driver’s License and access to a reliable vehicle. Available to work occasional evenings and weekends for CAP events.
